* one  piece of cardstock in a tag shape

  or a big tag

* two tiny tags

* glue

* a 3D figure

* paper or fabric flower

* selfadhesive lace

  or lace and a glue gun

* ovale shape die cut

* ovale shape old picture

* gems

* scissors 

* autumn color paints

* paper or fabric leaves

* babywipes
